Manish Patel, leveraging his IT and hospitality industry background, has overseen the growth of Avance Care since its inception in 2008. He orchestrated the establishment of the clinically integrated system of comprehensive primary care practices and enrollment of 60,000+ patient base while maintaining profitable revenue growth. Manish was the architect of Avance Care’s unique operating model, information technology, and workflow systems that enable the company’s patient-centered primary care delivery system that naturally supports value-based healthcare. He prides himself on being the company’s culture coach where he works to promote patient, provider, and staff satisfaction, and efficient operation.
Pratik V. Desai, MD, FACC
Owner & Co-Founder
Dr. Desai has been a practicing cardiologist since 2000. He co-founded Avance Care with CEO Manish Patel in 2008, with a vision to change healthcare by offering convenient, accessible and comprehensive primary care. He believed that overall healthcare cost could be reduced and outcomes improved, if primary care was convenient and comprehensive. Being a practicing physician, he is sensitive to the demands placed on providers and ensured that from day one, a core tenet of Avance Care was ensuring appropriate work-life balance as well as enabling high quality care supported by more patient time and enhanced support systems.
Joanne Fruth, MD
Chief Medical Officer
Dr. Fruth is a Fellow in the American Academy of Family Physicians with over 25 years of clinical experience. She brings her love of teaching to her role as Chief Medical Officer at Avance Care. Dr. Fruth encourages medical professionals to adhere to Dr. Atul Gawande’s research that “checklists saves lives” and to apply these principles to primary care. Physicians and advanced practice providers are inspired to systematically collect the grains of sand that comprise superior preventative and chronic care, then use their individuality as the cement that builds the foundation of an individual’s good health.
Chief Financial Officer
Levi joined Avance Care from the investment banking industry. He met the founders in 2012 and served as an early advisor, later joining as CFO in 2014. In addition to overseeing the financial health of Avance Care, he directs the company’s growth and corporate development efforts. In that time, Levi has directed a growth strategy resulting in a tenfold increase in revenue and a quintupling in the number of Avance Care locations, while maintaining positive cash each of those years.
Chief Technology Officer
Prashant Dangash is responsible for focusing on technologies that will grow the organization externally and continue to push the organization from technology enabled to technology driven. He has decades of experience in building and implementing Enterprise IT solutions across various industries and managing large global engineering organizations. His charter is to transform Avance Care from a tech-enabled to a tech-driven health care system that uses cutting edge technology to serve patients better, drive efficiency and generate better health outcomes. He holds a graduate degree in business administration from California State University and an undergraduate degree in engineering from University of Pune.
Frank Manole, DNP, MBA, FAANP
Vice President of Operations
Dr. Manole is a business leader, accomplished clinician, and educator. He is an active fellow of AANP, and serves as North Carolina AANP state representative. He has held teaching appointments at Rutgers University, UNC Chapel Hill, and serves as faculty/content expert for ANCC. Dr. Manole also oversaw clinical operations for over 40 clinics in his role of senior practice manager for CVS Health – MinuteClinic. His goal at Avance Care is to develop and execute strategies and sound operations that will change the landscape of healthcare, and improve access to quality, affordable care to all our communities.
Vice President of Finance and Accounting
Brett Farabaugh comes to Avance Care with over 30 years of experience in finance and accounting. His diverse experience spans multiple industries with an emphasis on entrepreneurial companies in the public and private arenas. Most recently, Mr. Farabaugh served as Interim Chief Financial Officer of TransEnterix, Inc., a public medical device company that is digitizing the interface between the surgeon and the patient to improve minimally invasive surgery. Mr. Farabaugh has served as CFO of Tryton Medical, Inc., a developer of coronary stents based in Durham, North Carolina from 2008 to 2019, and as CEO of Tryton Medical since August 2019. Prior to Tryton Medical, Mr. Farabaugh served as CFO of Strikeiron, Inc., a high-growth information technology company, from 2006-2008, as Vice President of Finance for Icoria, Inc. (formerly Paradigm Genetics, Inc.), a public biotechnology company, from 2004-2006. Mr. Farabaugh also held various roles at Nuada Pharmaceuticals and PricewaterhouseCoopers LLP. Mr. Farabaugh holds a B.S. in Commerce with a concentration in Accounting from the University of Virginia and is a licensed CPA.
Reynolds (Tad) Clodfelter, Psy.D.
Vice President of Wellness Services
Dr. Clodfelter is a licensed, clinical psychologist in the state of North Carolina and a behavioral health and addictions specialist. Dr. Clodfelter is a proven leader in behavioral health, addictions treatment and strategy, and has guided companies and many individual leaders to grow their clinical businesses and increase market share. He promotes whole-person health and believes that an integrated approach to healthcare and treatment is vital to sustainable clinical results. Accordingly, Dr. Clodfelter believes mental health, nutrition, and spiritual wellness are critical components of overall health and wellness. As Vice President of Wellness Services, Dr. Clodfelter targets high-quality behavioral healthcare solutions and policies to grow behavioral, nutrition, and psychiatry services for our patients.
Ashwin A. Patkar, MD, MRCPsych, DFASAM, DFAPA
Chief of Avance Psychiatry
Dr. Ashwin A. Patkar is Chief of Avance Psychiatry and Adjunct Professor of Psychiatry at Rush University Medical Center, Chicago. He was formerly Professor of Psychiatry and Community and Family Medicine at Duke University Medical Center, NC.
Dr. Ashwin A. Patkar Patkar has over 25 years of clinical and scientific experience in psychopharmacology, addiction, dual diagnoses, and integration of medical care for psychiatric disorders. Dr Patkar is certified in psychiatry, addiction psychiatry, addiction medicine, and pain medicine. He has published over 250 scientific articles, 6 book chapters, and has given over 400 presentations and invited lectures at national and international meetings. He is a distinguished fellow of the American Society of Addiction Medicine (ASAM), distinguished fellow of the American Psychiatry Association (APA), president of North Carolina Chapter of ASAM, the past president of the Indo-American Psychiatric Association, and member of the American Academy of Addiction Psychiatry, American Academy of Pain Medicine, Indian Psychiatric Society, Royal College of Psychiatrists, and World Psychiatric Association.
Vice President of Value-Based Care
Frank is a healthcare industry thought-leader in value–based care. Frank created the vision and strategy for Anthem Inc.’s value-based care solutions that offered holistic win-win relationships between payers and providers for total cost of care value programs. Frank oversaw the growth and operations of the program scaling to tens-of-thousands of providers serving millions of attributed patients. Frank also led content creation for Population Health Analytics tools and Clinical Decision Support at Anthem, Allscripts, and Zirmed to enable provider success in accountable care.
Frank is a champion of primary care as a key to transforming clinical and financial healthcare outcomes evangelizing through public speaking events, provider advisory committees, and working with organizations such as the Primary Care Collaborative, AAFP, and CMMI to create innovative value-based models. Frank understands the need for payer-provider partnerships and payer investments in progressive payment models as fuel for practice growth, productivity, and outcomes.
